Hazelwood History & Mission
|
Hazelwood Center is a licensed, state-owned and operated Center for individuals who are developmentally disabled functioning within the most severe/profound range of retardation. In addition, Hazelwood clients are primarily non-ambulatory with physical problems that include but are not limited to, scoliosis, cerebral palsy, muscular dystrophy, multiple sclerosis, osteoporosis, spinal bifida, Downs Syndrome, epilepsy and skeletal contractures. |

| Quality with Pride |
Since 1971, Hazelwood's 600 plus staff have provided "Quality with Pride" total care for its approximately 200 clients. These Kentucky citizens are provided innovative long-term care techniques that result in a lifestyle as unrestricted and healthy as possible. Hazelwood Center is their permanent home and total care is provided 24 hours per day, seven days per week. |

Need for a Dedicated Area
|
Hazelwood center has no dedicated space large enough to accommodate the client and family activities. The multipurpose room currently used offers no privacy and is in high demand for other client activities. Supplies and materials must be set up, taken down and stored away for each event. Special events, such as memorials for deceased clients do not meet normal privacy requirements for the family, friends, and staff. |

| A Project for All |
Hazelwood needs an area designated as a private area to accommodate reflection, mourning and special events for the patients, staff, family members, patrons and guests. |

| About the Project |
The addition will be an approximately 2,949 square foot Annex to the Main Hazelwood Residents Hall. The addition will meet all Kentucky State Government facility requirements since the State will subsequently accept title to and maintain the Annex. Dedicated space will provide for private family and caretakers' reflection, public non-denominational events, and visitors. The Annex will also house the Centers two full time chaplains and their resource materials. Handicapped egress and rest room facilities represent a very significant cost premium over normal construction costs. Over half of the Centers patients are bedridden with all others principally confined to wheelchairs. |

| Ways to Give |
Our singular, short-term purpose is to build the Annex, so gifts should be by cash or readily negotiable securities. Most gifts are made in cash, either personal or corporate check. A gift of appreciated stocks, bonds or other negotiable securities is one of the most advantageous ways to give. A transfer of these securities to the fund can save on capital gains tax and result in a charitable deduction for the full market value. |

Please Note: The fund is staffed entirely by volunteers and has no salaries nor recurring administrative expenses except nominal mailing and postage costs.
